Expert analysis and advice. When a worker attempted to work on an air release valve, the valve blew loose from a high pressure reclaimed water line severely injuring the worker. Reviewed the reports from OSHA, the agency and other parties. Observed in photographs taken just before the accident occurred that the air release valve was leaning.
